Guidelines for the Petroleum and Geothermal Energy Resource (Resource Management and Administration) Regulations 2014 Table 2: Generic Consequence Classification Table Showing Indicative Consequence ‘Zones’ and Examples of Common Qualifying Terms for Impacts If the information contained in the written notification is adequate and the proposed changes are acceptable to the Department, the Department will duly notify the title holder and consider the written notification to be part of the approved WMP. For changes outside this scope, such as change to an activity resulting in, or related to, moderate or higher additional or modified risks, a Management of Change document may be required. 3.3.4 Variation of a WMP A title holder may apply for approval of a variation of a WMP. The application must be accompanied by the varied WMP. As stated previously in 3.1.1, if a further well activity is proposed, then a well activity program should be submitted to the DMP and approved before the activity can occur. The approved program will be appended to the original WMP. The WMP is varied by the attachment of successive approved well activity programs. A title holder must also apply to vary a WMP if there is a change in the understanding of the geology or reservoir that may have a significant impact on a well activity. An application for variation of a WMP must also be made if there is the occurrence or potential occurrence of a significant new detrimental risk to well activities related to the WMP. Risk analysis, management and mitigation should be addressed in the application. In the case where a proposed further well activity has different or altered risks that have not previously been addressed in the WMP then the analysis, management and mitigation of these risks should be included in the proposed further well activity program which, once approved, will be attached to the WMP. Where Management of Change is used to address changes in an approved well activity, an approved Management of Change document will vary the WMP by being attached to it. The Department, having already approved the WMP being varied, will assess the variation document only, but will ensure that it is consistent with the original WMP, before issuing approval of the variation. The assessment timeframes in Section 3.2.3 also apply to bridging documents. 3.3.5 Document Control A WMP must be clearly identified with an appropriate title, document number, date and revision number. The revision number is particularly important, as there may be several iterations of the document during the assessment process. As the approved revision constitutes a legally binding agreement between the Department and the title holder, the revision that is finally approved by the Department must be clearly identified. The WMP should be kept up-to-date, as should all supporting documentation. Under regulation 67 of the m (RMA) Regulations, it is required to keep a record of all documents. Note that a WMP is valid for 5 years after initial approval, whether or not it has varied since the initial approval. If the WMP is still required after that time (i.e the well has not been plugged and abandoned) a new WMP must be resubmitted in good time to ensure that a valid WMP is always current. The Department should be consulted prior to submission to discuss the title holder’s approach to risk classification for a given activity. 3.3.3 Written Notification The Department may agree to written notification to enable the efficient processing of minor updates to an approved WMP due to a change to the activity which results in minor additional or modified risks. Written notification from the title holder to the Department may be in the form of a letter or email. The changes that may be processed in this way are limited to: • a new activity within the scope of an approved WMP that relates to minor risks covered in the approved WMP; • a change to location or operational details previously described in an approved WMP, where the risks associated with the change are addressed in the approved WMP; or • a change to location or operational details previously described in an approved WMP that results in minor additional or modified risks not covered in an approved WMP. www.dmp.wa.gov.au | 5 Lower third of consequence levels Consequence Low Moderate Catastrophic Reportable if unplanned
