This stack-up of conservatism results in an extremely robust actuation system at the expense of cost, weight, and potentially fuel system heat load if higher fuel pressure is utilized to provide the actuation capability. Resolution of interface uncertainties does not normally occur until a full engine test is performed. By the planning of success-oriented engine development programs that assume minimal design iterations, the PW (and therefore HS) culture has fostered this conservatism by punishing technical shortfalls. Risk taking is only rewarded when successes are achieved. Within the past decade or so, increased focus has been placed on making risk mitigation a normal part of development programs rather than as the result of the occurrence of an unforeseen program issue. Tools have been developed and deployed across airframers, propulsion system contractors, and throughout the supply base. A risk management tool used by Pratt & Whitney aids in the assessment of risk by assessing the two factors which comprise risk, probability of occurrence and severity of consequence. The probability of occurrence is estimated by comparing the particular issue to a list of criteria based on the type of risk such as supplier maturity, requirement uncertainty, design maturity, etc. This is assessed as a high, moderate, or low probability. The consequence is then assessed in three separate areas - performance, development cost, and program schedule with choices of significant, moderate, or minor. A mathematical formula combines the four scores together to produce a single value between 0.1 and 0.9. The weighting factors in the formula can be adjusted to put more emphasis on a particular attribute (such as schedule) if deemed necessary by the program. Any particular item that exceeds a predetermined risk threshold (0.6 for example) is tracked by the program risk management team and requires regular statusing.
